Welcome to Wandsworth PCN, a Primary Care Network of six surgeries serving about 50,000 patients. Our mission is to provide local, proactive, personalised, and integrated healthcare with a dedicated clinicians as shared resources including paramedics, pharmacists, Physicians Associates, and Social Prescribers in collaboration with St George’s Hospital Trust, community pharmacy teams, District Nurses, and other providers.

Early May & Spring Bank Holidays 2025 – Wandsworth Pharmacy Opening Times
Chatfield Health Care will be closed on 5th May 2025 for the May Day bank holiday and also closed on the 26th May 2025 for the Spring bank holiday.
